Education, health and care plans (EHCP)
Is for children and young people aged up to 25 who have complex special educational needs and disabilities and who need more support than is available through special educational needs support.
Identifies educational, health and social needs
It sets out the additional support to meet those needs for a child or young person to achieve their best possible outcomes.
Specifies how services will work together to meet a child or young person's needs and achieve agreed outcomes ( based on the child or young person’s needs and aspirations).
It’s a legal document ( Children’s and Family Act, 2014) starting from a child's early years, it and can remain in place until a young person is 25.
Is used to secure the provision that best meets the outlined needs.
EHCP has replaced Learning Difficulties and previous SEND reports.
Is acting to support the individual’s learning and future employment, home and independence, friends and access to community groups, health and well being.
